Documentation Guidelines

The Office of Accessibility Services has developed the following documentation guidelines to assist students in providing appropriate documentation regarding their disabilities. Students are encouraged to share these guidelines with their appropriate medical professionals.


Documentation Guidelines for Accessibility Services

Students requesting accommodations due to a disability must provide documentation from a licensed professional who is qualified to diagnose and treat the condition.

Documentation Requirements:

– Must be typed or printed on letterhead from the provider or agency.

– Handwritten notes or prescription pad notes will not be accepted.

How to Submit:

Documents should be uploaded when completing the online registration form. If you are unable to do so, you can email the form(s) to accessibility@mcneese.edu or bring them by our office located in BBC room 205A.

Required Information for the Documentation Letter:

1. List the primary diagnosis, date of diagnosis, and any additional diagnoses if applicable.

2. Describe your professional relationship with the student, including:
   – Type of treatment provided
   – Date of the most recent appointment

3. Explain how the disability significantly affects the student’s ability to fully access academic activities or housing, including specific functional limitations.

4. Include your full name, title, and your signature.


Definition of Disability:

An individual with a disability is defined by the ADA as a person who has a physical or mental impairment that substantially limits one or more major life activities.

Examples of major life activities include:

Caring for oneself, performing manual tasks, seeing, hearing, speaking, eating, sleeping, walking, standing, lifting, bending, breathing, learning, concentrating, thinking, communicating, and the operation of major bodily functions.
