Immunization Compliance

All first-time students attending McNeese must complete and return an Immunization Compliance Form.

Students cannot register for classes until proof of immunization or a vaccination waiver is received. Once the Admissions Office receives proof of immunization, Health Services will review and record the immunization records and electronically transmit them to the Louisiana Department of Health and Hospitals.

If it is determined that vaccinations are required, a registration hold will be placed on the student’s record, and the student will be contacted regarding the required vaccination(s).

  • HIPAA is the federal privacy standard that protects patients’ records and other health information provided to healthcare providers, health plans, doctors, and hospitals.

  • This form provides more information about your healthcare privacy rights and how to transfer or release medical information to authorized personnel or institutions.
  • Parental consent is required for students under the age of 18. If you are under 18 and will be seeking medical care at Health Services, please have your parent or guardian sign and submit this form.
  • This form reviews your rights as a patient seeking services at the Health Services facility.
  • STDcheck.com is a leading online STD testing provider now offering free HIV testing to McNeese State University students.

  • To get a free, confidential HIV test, go to www.STDcheck.com/universities and select Louisiana and McNeese State University. You can then use your student email to check out and visit our local testing center (LabCorp on Nelson Road) to get tested. Results are delivered within 24 hours.
  • Calcasieu Community Clinic is a not-for-profit, free health care clinic located at McNeese State University’s Juliet Hardtner Hall College of Nursing.

  • The clinic provides free ambulatory medical care and pharmaceuticals to the low-income, working uninsured in the five-parish area of Imperial Calcasieu, including Allen, Beauregard, Calcasieu, Cameron, and Jefferson Davis.
  • Healthy Louisiana is Louisiana’s name for its new expanded Medicaid program, which provides health insurance coverage to more than 350,000 working Louisianans who could not otherwise afford health care.

  • Many students qualify for Medicaid. Follow the link above to find out more about applying and the services offered to those who qualify.

Ms. Anna Grace Bizzle is an Academic Advisor for the College of Nursing and Health Professions. She earned her Bachelor’s and Master's degrees in Secondary Education from Mississippi State University. Her favorite part of advising at McNeese is being a resource for students as they get used to the expectations of college and helping students pursue their degrees at such a wonderful school.
