Specify how frequently the employee and supervisor will meet to discuss the effectiveness of the telework arrangement.
All University equipment removed from campus must be secure at all times. An Equipment Entrustee Form must be completed and approved by the supervisor prior to removing any equipment from campus. A copy of the completed and signed form must be attached to the copy of this agreement that is maintained by the supervisor.
All University documents, materials, and data that are removed from campus are the responsibility of the employee and must be kept confidential and secure at all times.
