Financial Aid Document Examples

The Smart Choice

Academic Transcript

  • Copy of your OFFICIAL academic transcript from your high school and/or previously-attended college/university.
  • We are unable to accept unofficial transcripts (e.g., screenshots, photos, or scans) and require an official transcript.
  • This can be sent as a PDF attachment from you or the school itself, or dropped off in person at Student Central if you requested a paper copy. 

Birth Certificate

  • Copy of your official birth certificate.
  • This can be sent as a PDF scan, an image, or dropped off in person at Student Central.

Citizenship

  • Copy of your United States passport or copy of your Certificate of Citizenship/Naturalization.
  • This can be sent as a PDF scan, an image, or dropped off at Student Central in person.

Custody Papers

  • Copy of official court documents proving custody.
  • This can be sent as a PDF scan, an image, or dropped off at Student Central in person.

Death Certificate

  • Copy of official death certificate.
  • This can be sent as a PDF scan, image, or dropped off at Student Central in person.

Divorce Documents

  • Copy of final divorce decree OR petition for divorce with clerk of court stamp and filed date OR proof of separation for past 6 months (which is utility bills with separate physical addresses).
  • This may be sent as a PDF scan or image, or dropped off at Student Central in person.

Marriage Certificate

  • Submit a copy of the official marriage certificate.
  • This can be sent as a pdf scan, image, or dropped off at Student Central in person.

Social Security Benefits Document

  • Submit a copy of Form SSA-1099, also known as the Social Security Benefit Statement.
  • This can be sent as a PDF scan, an image, or dropped off at Student Central in person.

Social Security Card

  • Copy of your official Social Security card.
  • This can be sent as a PDF scan, an image, or dropped off at Student Central in person.

W-2 Forms

  • Copy of your W-2s. Please submit all applicable W-2s for the total year.
  • If you do not have access to your W-2, please see the Tax Form Quick Reference for other acceptable documents.

Form 1099-R

  • Submit a copy of Form 1099-R.
  • This can be sent as a PDF, scanned image, or dropped off at Student Central in person.

1040-X Amended Tax Return Form

  • Submit a copy of your Form 1040-X, also known as an Amended Tax Return.
  • This can be submitted as a PDF scan or image, or dropped off at Student Central in person.
  • If you do not have access to your tax return, please refer to the Tax Form Quick Reference for other acceptable documents.

Nonresident (U.S.) Tax Return

  • Submit a copy of your U.S. Nonresident Alien Income Tax Return.
  • This can be sent as a PDF, scanned image, or dropped off at Student Central in person.

Tax Returns

  • A SIGNED COPY of your official tax return.
  • There must be a signature on the signature line; failure to sign will result in an incomplete form and can cause delays in your financial aid.
  • This can be submitted as a PDF scan or image, or dropped off at Student Central in person.
  • You may also be required to submit all associated tax schedules and 1098s/1099s, if applicable.

Ashley Bass

I am a McNeese graduate with a bachelor's degree in Family and Consumer Science and a concentration in Child and Family Studies. As a student, I competed on the rodeo team and served on the SGA board. I earned my master’s degree in 2022 from the University of South Dakota in Educational Administration and Leadership with a concentration in Adult and Higher Ed. I have over 13 years of experience in adult education and 20 years of educational experience.

My favorite part of being a Freshman Advisor is helping students navigate their new life routine as a college freshman by offering support with in-person office visits or by email to help students meet their goals. I always remind students of my favorite BB King quote – “Education is one thing no one can ever take away from you.”  I believe that supporting students is vital to a student's success.

In my spare time, I raise registered American Brahman cattle, and I enjoy all the "country girl" things like hunting, fishing, growing a garden and cooking.
