2020-2021 Independent Verification Worksheet

  • Your FAFSA was selected by the U.S. Department of Education for a review process called "Verification."  Verification must be completed before your financial aid application can be completed.  Do not leave anything blank on this form.
  • List the people in your household, including:
    • yourself, and your spouse, if you have one, and
    • your children, if you will provide more than half of their support from July 1, 2020 through June 30, 2021, even if they do not live with you, and;
    • other people if they now live with you, and you provide more than half of their support and will continue to provide more than half of their support from July 1, 2020 through June 30, 2021. 
  • Number in College: Include in the space below information about any household member, excluding parents, who is, or will be enrolled at least half time in a degree, diploma, or certificate program at an eligible postsecondary educational institution any time between July 1, 2020 and June 30, 2021, and include the name of the college.
  • If you need more space, attach a separate page.
  • Note: We may require additional documentation if we have reason to believe that the information regarding the household members enrolled in eligible postsecondary educational institutions is inaccurate.
  • If you or your spouse indicated that child support was paid on the FAFSA.  Please complete the information below.
  • If you cannot use or choose not to use IRS Data Retrieval on the FAFSA then submit a SIGNED copy of your 2018 Federal Tax Return (1040 including all schedules).
