Meeting Room Request Form


This form should be submitted for meetings held in Holbrook Student Union reserved spaces.  This form is not acceptable for a reservation identified as a Special Event.  If you are unsure about your request, contact the Student Union & Activities Office at 337-475-5609 between 7:45am - 4:30pm, Monday-Friday.

You will receive an email or phone call from the Student Union & Activities Office confirming or denying your reservation.

NO FOOD OR DRINKS are allowed in the Mariner, Mallard, or Gallery Rooms.
Are you requesting a consecutive reservation?
Please note that you can only request a consecutive reservation through the following month. For instance, if it is August, you can reserve consecutively through September. In September, you will be able to reserve consecutively through October.
Will you be using the LCD monitors, projection, or audio equipment?
If you will be using any technology, a technical request form must be completed to be eligible to use any of the equipment.
I, the above signed, reserving the above meeting room(s), agree to the following: • This is a meeting and not a Special Event Request. If it is I am aware that this is not the form that should be completed for a special event request. Any questionable activity may result in the group listed above losing room rental privileges. • All meeting rooms being used are to remain in the same condition in which it was issued. • If meeting room(s) are not cleaned after use, a clean-up fee of $75.00 will be charged. • I agree to be fully and solely liable for all damages to, or loss, or destruction of, from any and all cases, including, but not limited to accidents, fire, theft, and vandalism that occur during my use of the meeting room(s). • I agree to report any changes in information pertaining to this meeting to the Union Office immediately and prior to the meeting. • All reservations are subject to the final approval of Union Office Administration.
