During evacuation, did
a. All alarms function properly?
b. Occupants follow emergency evacuation plans as posted?
c. All self-closing doors close?
Were bicycles, motorcycles, vehicles, furniture, appliances, etc., obstructing any means of egress (exit doors, corridors, stairs)?
Did all building occupants participate?
Did anyone remain in the building?
Did any unauthorized personnel enter the building?
Did building occupants respond in a timely manner by evacuating their areas as rapidly as possible?
Were elevators used?
Were elevators shut down?
Did building occupants evacuate by the nearest exit?
After evacuation, were building occupants directed to stand at least 50-100 feet away from the building and/or advised to report to a designated location?
Was the drill scheduled at an appropriate time period during the semester so that it did not conflict with student holiday and/or vacation periods?
Were adequate Fire Response Personnel (FrePs) available to check rooms or other-wise assist in building evacuation?
Were all floor reports filled out and used in this report?
Do they reside/work in that building?
Was Campus Police present?
Was the University Safety Officer present?
